Maple Family Health Team, in partnership with the Frontenac, Lennox & Addington Ontario Health Team (FLA OHT) and other community partners, is expanding its interdisciplinary primary care services through new provincial funding aimed at increasing access to comprehensive, team-based healthcare for people in Kingston and the surrounding area who do not currently have a family physician.

We are seeking collaborative, patient-centered professionals who are excited to help shape an cutting-edge model of primary care while working alongside colleagues from a wide range of health disciplines.

About the Position

We are seeking Registered Practical Nurses to join our interdisciplinary primary care team. This position offers an exciting opportunity to help shape and expand an innovative model of team-based primary care for unattached patients in our community.

Working collaboratively with Nurse Practitioners, Registered Nurses, Psychologists, Mental Health Counsellors, Registered Dietitians, Community Service Workers, other healthcare professionals and administrative professionals, you will provide education on prevention and management of chronic disease to patients of all ages.

What you’ll do

- Deliver education and counselling in chronic disease prevention and management for chronic conditions in keeping with clinical guidelines and best practice.
- Administer immunizations and perform procedures including ear irrigation, suture removal, and BP Tru.
- Conduct routine assessments and screenings using evidence-based guidelines.
- Maintain accurate, timely documentation in the electronic medical record.
- Participate in team meetings and education opportunities.
- Work closely with members of the healthcare team to coordinate patient care.
- Assist patients in accessing community resources and support services as appropriate.
- Participate in case conferences, team meetings, quality improvement initiatives, and collaborative care planning.
- Contribute to the ongoing development and growth of the clinic’s preventive care and chronic disease management services.





Qualifications:

- Current registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse.
- Experience in primary care, community health, or patient health teaching with all ages.
- Proficiency in electronic medical records and digital tools for care delivery.
- Commitment to confidentiality, professional standards and evidence-based practice.
- Knowledge of community resources within Kingston and area.
- Ability to practice in a second language (asset).

Information about the position

- All providers in the Health Hub will participate in a rotating schedule for occasional evening availability (one-to-three evenings per month).
- HOOPP, Dental and Extended Health benefits begin immediately.
- Full COVID vaccination is required unless there is a religious or medical exemption. Proof of Measles immunity is required.
- Compensation is aligned with Ministry of Health funding for Registered Practical Nurses in a Family Health Team, which, for this fiscal year, is a maximum of $55,960 per year.
