This part time role involves planning, implementing, and evaluating individual and group activation programs assigned by the Supervisor. You will transport residents, monitor their condition, and maintain accurate records as part of your responsibilities. Working cooperatively across departments is crucial to meeting resident needs.
Key Responsibilities:
• Develop and implement individualized activation programs
• Ensure cleanliness of activity areas for optimal use
• Monitor and report on residents’ behavior and condition




• Participate in program development and revisions
• Record and communicate resident participation data
Requirements:
• Completion of an Activation/Gerontology Diploma or Degree
• Strong communication and documentation skills
• Experience in care planning and resident assessments
• Independent and effective time management abilities
• Knowledge of working with the geriatric population
