Key Responsibilities: • Engage in community visits to support patient care • Provide ethical and protected nursing services • Assess patient needs with relevant tools • Implement and review care plans regularly • Build lasting relationships with patients through collaboration
Requirements: • Good standing as a Registered Practical Nurse with CNO • Valid driver's license and vehicle required • Experience in community nursing advantageous • Reliability and flexibility in patient travel • Availability for on-call coverage as required
