Become a Hospice Social Worker at Kensington Health and make a difference in patients' and families' lives in Toronto. Assist with psychosocial support and care planning during critical end-of-life moments.

As part of the interdisciplinary team, you will conduct in-depth psychosocial assessments and collaborate on individualized care plans. Your role will include providing therapeutic support for grief and loss while ensuring that residents receive equitable care. Engage in various initiatives that prioritize quality improvement and community resource access.

Key Responsibilities:



• Conduct psychosocial evaluations to identify support needs • Develop tailored care plans with interdisciplinary input • Provide emotional support and counseling during critical times • Facilitate group sessions and family meetings for support • Monitor residents' rights and advocate for their needs

Requirements: • Master’s Degree in Social Work from an accredited program • Minimum two years’ experience in palliative or hospice settings • Proficiency in Microsoft Office and Point Click Care • Robust interpersonal skills for team and family dynamics • Active license or eligibility as a Registered Social Worker
