Provide and coordinate home health nursing services to diverse individuals, families, and groups
Assess, monitor, and make care planning and care coordination
Evaluate care plan interventions through ongoing reassessment and evaluation results
Ensure quality patient care by identifying and acting on factors affecting practice autonomy and care delivery
Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families
Support safe client care through development of knowledge and skills in continuous quality improvement
Inform the nursing supervisor of client activities, relevant concerns, or changes in condition or care plan
Submit appropriate documentation to the case coordinator
Maintain documentation standards according to the College of Nurses of Ontario
Work within ISDS to strengthen integrated care, promote interdisciplinary collaboration, and provide a seamless client experience
Requirements
Registered and in good standing with the College of Nurses of Ontario (CNO)
RPN diploma or RN diploma/degree from a recognized college or university
Hold a valid First Aid or CPR Certification
A valid Ontario driver's license (G License) and a reliable vehicle for traveling are required
Morning and day availability
Bi-weekly weekend rotation, from 8:00 a.m.-4:00 p.m. or 9:00 a.m.-5:00 p.m.
Effective written and verbal communication and documentation skills
Reliable attendance, punctuality, and availability
Commitment to lifelong learning and professional growth
Ability to apply critical thinking skills to provide client care
Membership of your Qualified Association preferred (RNAO/WeRPN)
Moderate wound care or venipuncture knowledge and experience is an asset
Acute Care and Medical-Surgical nursing experience is an asset
Knowledge of community nursing is an asset
Core Competencies Demonstrates expertise in home health nursing services, care planning, and quality improvement while ensuring compliance with the College of Nurses of Ontario standards. Strong communication and critical thinking skills are essential for effective client care and interdisciplinary collaboration.
