In this position, you will collaboratively plan and evaluate Person-directed treatment plans alongside Service Users and team members. Your role encompasses addressing physical needs and fostering emotional well-being through strong therapeutic relationships. Scheduling will be versatile to meet diverse operational needs.
Key Responsibilities: • Implement individualized programs promoting independence • Collaborate with teams for coordinated care delivery • Maintain accurate administrative records in AIMS • Promote and support System Users’ spiritual and emotional needs • Ensure transportation to appointments
Requirements: • Two years experience working with developmental disabilities • Diploma in Developmental Social Work or equivalent • Current First Aid & CPR certification • Driver’s license that meets Ontario standards • Criminal Records Check required
