The Assessor Coordinator degree will ensure that the case management services are consistent with the policies, vision, mission and philosophy of the SHA. The position will coordinate the utilization of the electronic assessment system RAI-MDS (hereafter referred to as MDS)



as the screening tool for continuing care eligibility and placement and alternative level of care services placement within the SHA.
