- Provide and coordinate home health nursing services to diverse individuals, families, and groups
- Assess, monitor, and make care planning and care coordination
- Evaluate care plan interventions through ongoing reassessment and evaluation results
- Ensure quality patient care by identifying and acting on factors affecting practice autonomy and care delivery
- Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families
- Support secure client care through development of knowledge and skills in continuous quality improvement
- Inform the nursing supervisor of client activities, relevant concerns, or changes in condition or care plan
- Submit appropriate documentation to the case coordinator
- Maintain documentation standards according to the College of Nurses of Ontario
- Work within ISDS to strengthen integrated care, promote interdisciplinary collaboration, and provide a seamless client experience

Requirements

- Registered and in good standing with the College of Nurses of Ontario (CNO)
- RPN diploma or RN diploma/degree from a recognized college or university
- Hold a valid First Aid or CPR Certification
- A valid Ontario driver's license (G License) and a reliable vehicle for traveling are required
- Morning and day availability
- Bi-weekly weekend rotation, from 8:00 a.m.-4:00 p.m. or 9:00 a.m.-5:00 p.m.
- Effective written and verbal communication and documentation skills
- Reliable attendance, punctuality, and availability
- Commitment to lifelong learning and professional growth




- Ability to apply critical thinking skills to provide client care
- Membership of your Professional Association preferred (RNAO/WeRPN)
- Moderate wound care or venipuncture knowledge and experience is an asset
- Acute Care and Medical-Surgical nursing experience is an asset
- Knowledge of community nursing is an asset
