The Alex, a community-based not-for-profit, is looking for a skilled Registered Nurse for its Pathways Assertive Outreach program.

In this contract position, you'll work within a team-oriented multidisciplinary team to deliver medical interventions and support services. Your expertise will aid participants in achieving stability and wellness, while emphasizing trauma-informed and person-centered care principles. Key Responsibilities:
Provide assertive outreach services to individuals with significant mental health issues
Conduct assessments and plan supportive care for program participants




Collaborate with the Housing First Medical Clinic for care coordination
Complete monthly assessments and necessary documentation
Assist participants with accessing health resources and advocacy Requirements:
Bachelor's degree in Nursing
Active registration with CRNA
Minimum two years of nursing experience
Competency in venipuncture and specimen handling
