Enhance geriatric care as a Reactivation Worker in the Emergency Department. Support older adults with mobility, functional independence, and care transitions in collaboration with the GEM team. As part of the Geriatric Emergency Management Program, the Reactivation Worker plays a pivotal role focusing on therapeutic interventions and discharge planning.
This role requires an OTA/PTA designation and involves executing reactivation plans under the guidance of Registered Nurses and Social Workers. Your contribution will significantly impact patient outcomes and emergency department efficiency. Key Responsibilities:
Provide early mobilization and functional support to older adults
Facilitate restorative activities focused on strength and balance
Assist with ADLs such as transfers and toileting
Collaborate with the GEM team for timely discharge planning
Educate patients on secure mobility and falls prevention strategies Requirements:
Diploma/Certificate as OTA or PTA
Two years of experience with older adults in acute care
Knowledge of geriatric syndromes and recovery practices
Proficient in Microsoft Office and patient information systems
Excellent communication and interpersonal skills Contribute your skills in mobility support and care coordination to improve patient experiences in the Emergency Department.
