Requirements:
- Current registration as a Registered Nurse in Ontario
- Robust experience with seniors and care coordination
- Excellent communication and time management skills
- Sound clinical knowledge and delegation abilities
- Recent Vulnerable Position/Sector Screening (within 6 months)
