Become a pivotal part of the Canadian Mental Health Association as a Transitional Case Manager in Oxford, Ontario. In this full time, 12-month contract position, you will collaborate with individuals experiencing mental health challenges.

Conduct various client assessments for needs and barriers
Assist clients in setting and achieving personal goals




Develop and implement individualized rehabilitation plans

Diploma or degree in health or social services discipline
Valid G class driver’s license and access to a vehicle
Satisfactory Police Vulnerable Sector Check
