Participate in scheduled community visits for patient care
Assess patients’ needs using appropriate techniques




Execute and monitor customized care plans
Build rapport and trust with patients
Maintain and update qualified patient records Requirements:
Registration as a Registered Practical Nurse with CNO
Valid driver’s license and vehicle access
Experience in community care is advantageous
Flexibility for travel to patient homes
