This part time role focuses on delivering person-centred support, using your knowledge to guide clients through various pathways.

Your interactions will include face-to-face meetings, telephone calls, and virtual support to empower individuals in their community, ultimately fostering independence and reducing isolation. Key Responsibilities:
Connect clients to essential local services and groups

NVQ Level 2 in Health and Social Care or working towards it
