Key Responsibilities:
Conduct home safety assessments and equipment prescriptions
Develop fall prevention strategies tailored to individual needs
Implement rehabilitation plans for stroke and chronic conditions
Supervise occupational therapy assistants as required
Educate clients on available community resources Requirements:
Registration in positive standing with COTO
Strong communication and documentation skills
Experience in community practice is an asset
Valid G or G2 driver’s license for travel
