IWK Health in Halifax seeks a Family Care Coordinator Registered Nurse (FCC RN) to join the Nephrology team. You will deliver holistic, evidence-based care to nephrology patients and families, guiding planning, implementation and evaluation in collaboration with physicians and health teams.
The role covers hemodialysis, renal transplants, PD and other nephrology conditions, with in-person and virtual follow-ups.
