The Nurse Practitioner works to support the mission and vision of Neighborhood Healthcare (NHC) by providing excellent patient-centered care and services within a dynamic team environment. This role will provide general medical care and treatment to patients under the direction of providers. In addition, this role will assess training opportunities to promote team development.4 days a week 8am-5pm (32 Hour Week)2 days at Menifee location and 2 days at BeaumontResponsibilitiesPerforms physical examinations and preventive health measures within prescribed guidelines/instructions from providers and NHC practice protocolsOrders, interprets, and evaluates diagnostic tests to identify and assess patients’ clinical problems and health care needsRecords physical findings and formulates plan and prognosis based on patient’s conditionSubmits health care plans and goals of individual patients for periodic review and evaluation by providersPrescribes or recommends drugs or other forms of treatment to patients, such as physical therapy, inhalations therapy, or related therapeutic proceduresRefers patients to providers for consultation or specialized health resources for treatmentImpacts patient experiences by demonstrating courteous and helpful behavior and a commitment to accuracyTeam SupportShares accountability for overall patient health outcomes by working in coordination with care teamsOperates to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholdersResponds promptly and with caring actions to patients and employeesMaintains professional working relationships with all levels of staff, clients,
