As a Field Nurse Case Manager, you will play a critical role in supporting patients throughout their treatment journey within a Patient Support Program (PSP). Acting as a trusted healthcare partner, you will provide education, clinical support, case management, and therapy coordination while collaborating with patients, healthcare providers, pharmacies, and internal reimbursement teams. This role combines patient care, healthcare navigation, and case management to help improve patient outcomes, facilitate access to therapy, and deliver an exceptional patient experience.
The successful candidate will be passionate about patient advocacy, possess strong clinical knowledge, and excel at building relationships with healthcare professionals across British Columbia. Deliver patient-centered case management services in accordance with program protocols and clinical standards. Support patients throughout their treatment journey, including onboarding, education, adherence support, and ongoing follow-up.
Provide disease-state, therapy, and medication education to patients and healthcare providers. Collaborate with physicians, clinics, pharmacies, infusion clinics, and internal teams to coordinate patient care and facilitate access to therapy. Partner with Reimbursement Specialists to support funding requests, reimbursement navigation, prior authorizations,
and patient access solutions.
Conduct virtual and telephone-based patient follow-up, monitoring, and adherence support. Monitor patient progress, treatment adherence, therapy interruptions, and clinical outcomes in accordance with program requirements. Accurately document patient interactions and maintain complete records within program systems and applicable patient management platforms.
Identify and report adverse events (AEs) and product complaints in accordance with regulatory and company requirements. Participate in ongoing disease-state, product, and therapy-specific training programs. Attend educational events, conferences, and clinic meetings to support patient and healthcare provider engagement.
Build and maintain collaborative relationships with healthcare providers and clinic staff to ensure a positive patient experience. Maintain strict confidentiality and compliance with privacy regulations and company policies. Diploma or Degree in Nursing.
Experience providing direct patient care, patient education, healthcare coordination, patient support, or case management. Strong verbal and written communication skills in English. Proficiency with Microsoft Office applications, including Outlook, Word, Teams, and Excel. Strong organizational skills with the ability to manage multiple priorities in a rapid-paced environment.
Experience working within a Patient Support Program (PSP), pharmaceutical, specialty pharmacy, infusion clinic, reimbursement, or healthcare environment. Active nursing registration in additional Canadian provinces.
Experience supporting specialty therapies, biologics, oncology, neurology, immunology, rare disease, or chronic disease management programs.
Experience using case management platforms, CRM systems, or electronic medical records (EMR/EHR). Strong patient advocacy, relationship management, and problem-solving skills. Bilingualism in English and another language is considered an asset.

Working Conditions: Hybrid field-based role with occasional travel throughout British Columbia for clinic visits, healthcare provider meetings, patient education activities, conferences, and stakeholder engagement. Primarily remote/home-office based when not traveling. Frequent computer, telephone, and virtual meeting use required. Occasional evening or extended-hour support may be required based on patient and business needs. This is determined by several factors, including performance, experience and skills, equity, regular job market evaluations, and geographical markets.
