Rainbow Valley Community Health Centre (RVCHC) is in the Ottawa Valley in the village of Killaloe, Ontario. RVCHC is the only Community Health Centre in Ontario to be administered by a hospital, St.

Francis Memorial

Hospital (SFMH). The team is comprised of part time staff including Physicians, Nurse Practitioners, Registered Nurses, administrative staff and Social Worker. We are very proud of our high retention of staff and are continuously grateful for the amazing support of our community. As a result of internal movement at RVCHC, we are hiring a permanent full-time Social Worker.

The Rainbow Valley Community Health Centre

Social Worker functions as a member of the primary health team in the provision of assessment, individual and family counseling, case management and facilitation of groups operating within the values and objectives of the Centre.

The Social

Worker works closely within the inter-disciplinary team, providing consultation to members of the team. Provides information and advice to clients of the Centre and residents of the catchment area who are seeking support for mental health issues, and/or issues impacting on their ability to cope. Participates in case conferences, and provide case management to clients and families as required.

Contributes to the development of current programs that address community health needs by collaborating with Centre staff and other agencies to identify,



recommend and/or develop resources and materials and by participating in the delivery of programs as required. Provides leadership for appropriate community educational programs in collaboration with Centre staff and partner agencies. Works with clients to develop Coordinated Care Plan to meet RVCHC annual target.

Participates in the development of relevant policies, procedures and best practices for the Centre. Initiates and maintains partnerships with relevant community agencies and professionals. Promotes community awareness of and participation in the Centre’s programs.

Plans and participates in outreach activities related to primary health care. Works in a manner that preserves confidentiality, seeks to minimize risk, incorporates health promotion and recognizes the determinants of health. Respects and values the diversity of communities and individuals.

Contributes to and participates in the Continuous Quality Improvement Program by assisting to implement changes as required to improve services and enhance departmental efficiency. Accurately and consistently adheres to all Clinic and relevant Hospital policies and procedures. Complies with SFMH Ethics Statement and the Joint SFMH/VM Ethics Advisory Committee – Submissions Policy & Procedure.





As defined in the hospital Occupational Health and Safety Roles and Responsibilities, performs all work in a safe manner including infection control, client handling program, WHMIS, all emergency codes etc. Upholds the ethical and legal responsibility to maintain the confidentiality and privacy of client health information obtained while providing care, in accordance with the Personal Health Information Protection Act (PHIPA). Establishes and maintains respectful, collaborative and professional relationships that include relationships with colleagues, health care team members and employers.

A Registered Social Worker with a minimum of a Bachelor’s Degree and two to four years of counseling experience (or an equivalent combination of education and experience). Master’s degree preferred. Thorough knowledge of and proficiency in current assessment and psychosocial counseling techniques; Commitment to and knowledge of community-based health care, preferably in a rural setting; Sensitivity to people with a variety of health, social, cultural and economic needs.
