Your role will focus on improving outcomes for frail older adults in a collaborative environment. As part of this permanent full time position, you'll engage with various health and social care professionals to conduct frailty assessments and develop effective care plans. Your dedication to delivering high-quality, person-centred care will play a significant role in helping patients remain in their homes and avoid hospital admissions. Key Responsibilities:
- Perform detailed assessments for older adults facing frailty
- Aid in clinical decision-making and management guidance
- Enable safe discharges and admission avoidance
- Collaborate with multidisciplinary teams in health and social care
- Provide leadership and expert advice as needed
Requirements:
- Valid NMC UK pin and healthcare professional status
- Experience in urgent care or community services
- Exceptional clinical assessment and communication skills
- Proven leadership capabilities within a healthcare setting
- Postgraduate qualifications in advanced practice preferred
