Make a difference as an Extended Length of Stay Risk Assessor at Health Sciences North. This full time role focuses on patient assessments and coordinating Hospital to Home transitions. This permanent position within the Access & Flow department seeks a dedicated qualified to engage patients and evaluate risks associated with extended stays.
You will collaborate with interdisciplinary teams and stakeholders to ensure appropriate patient pathways for the Hospital to Home program. Your insights will lead to effective discharge planning and improved care delivery. Key Responsibilities:
Conduct initial Home First Philosophy discussions with patients
Complete assessments using InterRAI and other screening tools
Liaise with community support services for patient onboarding
Document outcomes and recommendations in patient charts
Engage in daily care team meetings and support cross-department communication Requirements:
Bachelor’s Degree in health-related field
Minimum two years’ experience in healthcare
Knowledge of risk assessment tools
Familiarity with elderly care and their caregivers
Training in Lean methodologies is a plus Drive positive health outcomes and enhance transitional care through this essential role.
