Family Service Toronto seeks a Case Manager for Alternative Level of Care (ALC) to provide intensive, time-limited case management for adults with developmental disabilities. You will coordinate discharge planning, services and supports to enable secure community living.

Reporting to the Manager, Developmental Services, this role requires collaboration with hospitals, families, and community partners, with solid advocacy and trauma-informed practice.
