This role is for straight day shift 830am to 430pm Mon-Fri

Must be Registered in good standing with the CLHA College and available to start asap.

Job Summary: We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our PRIMARY healthcare team. As an LPN, you will provide direct patient care, administer VACCINES , and assist in the Nurse Manager and Primary Care RN in the successful criteria needed for continued funding of the health services model via ISC. Your expertise and dedication will contribute to the overall well-being and comfort of our patients.

Duties

- Administer vaccines and treatments as needed per regional protocol
- Monitor patients' vital signs and report any changes or abnormalities
- Assist in the development and implementation of patient care plans
- Collaborate with healthcare professionals to ensure effective and coordinated patient care
- Provide emotional support and education to patients and their families
- Maintain accurate and up-to-date medical records
- Adhere to all safety protocols and infection control measures




- Assist with the coordination of patient admissions, transfers, and discharges

Skills
- Proficient in using electronic medical record systems, such as Epic
- Experience in long term care, pain management, senior care, dementia care, nursing home, hospice care is highly desirable
- Strong knowledge of medical terminology and healthcare procedures
- Excellent communication and interpersonal skills
- Ability to remain calm under pressure and handle emergencies with composure
- Attention to detail and strong organizational skills
