As a Nurse Practitioner, you will work independently under the full NP practice scope, contributing to comprehensive care for older adults. Your role includes conducting intake assessments, developing care plans, and coordinating patient flow while liaising with community providers to ensure continuity of care. You’ll also lead health promotion initiatives and participate in quality improvement efforts.
Key Responsibilities:
• Conduct intake assessments and medication reconciliation • Develop initial care plans with the interdisciplinary team • Coordinate patient flow and maintain accurate EHR documentation • Prescribe and monitor medications for primary and psychiatric care • Create discharge plans and support protected transitions
Requirements: • Master's degree in Nursing or BSCN with NP certification • Minimum of 5 years' experience in healthcare • Current Ontario registration in the Extended Class • Knowledge of relevant legislation is essential • French and English language skills required
