Reporting to the Director of Care, the Resident Care Coordinator plays an essential role in supporting a multidisciplinary team in delivering excellent care. You will conduct resident assessments, manage medication reviews, and facilitate care conferences with families and stakeholders. The position demands a high level of clinical judgment and the ability to work autonomously while collaborating with nursing staff to meet care standards.

Key Responsibilities:
• Support licensing and regulatory compliance efforts
• Conduct resident admission assessments as instructed




• Organize multidisciplinary care conferences and medication reviews
• Monitor nursing staff performance and patient care
• Attend to medication error reports and ensure follow-ups

Requirements:
• Must be a Registered Nurse in British Columbia
• Two years of relevant nursing experience required
• Current CPR and First Aid certification needed
• Knowledge in long-term care is an asset
• Solid critical thinking and communication skills are essential
