Southbridge Care Homes is seeking an RAI Clinical Coordinator in Seaforth, ON. The role focuses on managing MDS assessments, ensuring accurate documentation, and supporting the care team to deliver high-quality resident care.

The successful candidate will coordinate MDS completion, analyze data across multiple levels, and act as liaison with Health Zone, AH&W;, CIHI, and PCC. A RPN with CNO registration is required.
