In this role, you will support individuals in their homes, assist in medication reconciliation, and liaise with interdisciplinary teams. Key Responsibilities:
• Manage clinical aspects of drug distribution and dispensing
• Provide consultations and medication management in home settings
• Conduct medication histories and develop care plans




• Contribute to interdisciplinary care rounds and improvement projects
• Offer expertise in Ontario Health Team initiatives

Requirements:
• Graduate from an accredited Pharmacy Program
• Registered and in positive standing with the Ontario College of Pharmacists
• Preferred experience with older adults
• Strong communication and relationship-building skills
• Valid driver's license and access to a car preferred
