NHS Greater Glasgow and Clyde is seeking an Advanced Frailty Practitioner for the Home First Response Service. Your role will focus on improving outcomes for frail older adults in a cooperative environment.
As part of this permanent full-time position, you'll engage with various health and social care professionals to conduct frailty assessments and develop effective care plans. Your dedication to delivering high-quality, person-centred care will play a significant role in helping patients remain in their homes and avoid hospital admissions.
Key Responsibilities:
• Perform detailed assessments for older adults facing frailty • Aid in clinical decision-making and management guidance • Enable safe discharges and admission avoidance • Collaborate with multidisciplinary teams in health and social care • Provide leadership and expert advice as needed
Requirements: • Valid NMC UK pin and healthcare professional status • Experience in urgent care or community services • Exceptional clinical assessment and communication skills • Proven leadership capabilities within a healthcare setting • Postgraduate qualifications in advanced practice preferred
