CBI Home Health is looking to hire an Integrated Care Lead in Hamilton.
Salary Range: The salary range for this position is $70,000– $85,000 per year.

What you will do The Primary Nurse Clinician & Supervisor is a clinical leader who guides Nurses and Unregulated Care Providers in delivering advanced, best-practice care in home and community settings. Combining expert clinical care with leadership and mentorship, you will stabilize complex patients through comprehensive assessments, individualized care planning, and timely interventions, often within 24 hours, to improve outcomes, support care transitions, and help prevent hospital admissions. You will ensure seamless, high-quality care by identifying evolving patient needs, coordinating with interdisciplinary teams, and driving continuity of care through clinical excellence and effective communication.
As Primary Nurse
Clinician, you will also build team capability through coaching, training,
and the promotion of patient-centred care, quality improvement, and advanced community-based practice.

What you will need to be successful
- Registered Nurse in good standing with the College of Nurses of Ontario (CNO)
- 3 - 5 years of supervisory experience or equivalent clinical leadership experience in home and community healthcare.
- Advanced Clinical Expertise: Demonstrated ability to deliver acute and advanced care in the home and community setting.
- Strong Leadership Skill: Proven track record in team development, supervision, and fostering collaboration.
- Exceptional Organizational Skills: Capacity to manage time-sensitive visits, prioritize patient needs, and maintain comprehensive records.
- Commitment to Quality Care: Passion for delivering high-quality, safe, and patient-centred services.
- Robust proficiency with Microsoft Office Suite and experience with scheduling/ documentation systems.
- A valid Ontario driver's license, and access to a reliable, insured vehicle.
- Clinical Expertise: Strong assessment, care planning, and medication reconciliation skills.
- Problem Solving: Analytical and critical thinking to identify service gaps and implement improvements.
- Communication: Excellent verbal and written skills for interacting with diverse stakeholders.
- Organizational: Superior time management and attention to detail for managing competing priorities.
- Team Collaboration: Commitment to fostering interdisciplinary teamwork and patient-centred care.
