Reporting primarily to the Manager of Clinical Services, you will work flexibly within various programs, conducting assessments, developing treatment plans, and ensuring clients understand their participation rights. Your role is crucial in advocating for client access to additional resources as needed.
Key Responsibilities:
• Conduct and complete client assessments and treatment plans
• Provide education on mental health and coping strategies
• Support client integration into social and work environments
• Facilitate group programs and community connections
• Maintain accurate file management and reporting
Requirements:
• Degree in Social Work or Nursing with registration
• Two years of relevant experience with vulnerable adults
• Comprehensive knowledge of case management principles
• Proficient in clinical assessment tools
• Valid Ontario Driver’s License and vehicle access required
