The People We Support
We serve a remarkable community of individuals with diverse backgrounds and experiences. Since founding in 1987 as part of the Welcome Home program, we've been dedicated to supporting adults with intellectual disabilities as they transition from institutional settings to vibrant community living.

We provide 24-hour support to individuals with Autism Spectrum Disorder, FASD, mental health challenges, problematic substance use, and behavioural and sensory challenges, ensuring each person receives the care, respect, and opportunities they deserve.

Outreach Support Mentor Job Summary
As an Outreach Support Mentor, you will provide one-on-one support to individuals within the SIL, Outreach, and Respite programs, both in their homes and in the community. During your shift,



you will typically meet with 3–5 individuals.

Although you work one-on-one with the individuals you support, you are not alone. Outreach Support Mentors collaborate closely with other team members to ensure that the needs outlined in support plans are met and to empower individuals to achieve their goals.

Please Note: This role will primarily involve supporting individuals within female residences. To best meet the needs and preferences of the people we support, preference will be given to female candidates. This is considered a bona fide occupational requirement (BFOR).

Duties and Responsibilities

Connecting with community resources (food banks, clothing resources, etc.)

Attending medical appointments

Managing medication regimes

Grocery shopping

Learning how to take the bus

Bill payments and money management

Supporting individuals to acquire and maintain productive roles (employment, volunteering, programming, etc.)

Accessing programming

Recreational opportunities (skating, swimming, sporting events, etc.)

Group programming opportunities

Meal preparation and cooking

Cleaning and maintaining the home

Emotional support

Crisis support

Skills and Qualifications

High School Diploma (minimum)

Some post-secondary courses in the social services field (preferred)

2 years' experience working with adults with an intellectual disability

Knowledge of disabilities, disorders, and mental health issues

Excellent communication skills

Advocacy skills





Flexibility

Other Important Requirements

Valid Manitoba Driver's License with a satisfactory driver's abstract

Use of a safe vehicle for work purposes

Criminal Record Check with Vulnerable Sector Search

Adult Abuse Registry Check

Child Abuse Registry Check

Current CPR and First Aid certification

Experience providing personal care is an asset

Preference May Be Given To Candidates Who Have

Experience working with mental health

Experience working with addictions

Experience working within the justice system

Experience supporting individuals with co-occurring disabilities

Full-time opportunities are available with flexible scheduling between 10:00 a.m. and 8:00 p.m., Monday through Sunday.
