As a dedicated Assessor Coordinator, you will provide essential case management services, engaging in screening, planning, and monitoring the care needs of clients. The role requires prompt response to service requests and a collaborative approach with program teams. You will also maintain vital community resources and statistical data for program development.

Key Responsibilities:
• Perform screening and assessment of mental health service requests
• Assign and coordinate service responses based on client needs




• Respond effectively to crisis situations while providing support
• Collaborate with teams to enhance the intake program
• Keep detailed records, safeguarding client information confidentiality

Requirements:
• Relevant degree in Human Services or a health-related field
• Experience in working with mental health clients
• Valid Class 5 driver’s license
• Registration with Saskatchewan Association of Social Workers
• Excellent interpersonal and organizational skills
