The Health Information Management Professional will perform technical duties related to health information management. Additionally, organization, coordination and generation of reports related to technical analysis of health information is required.
Duties & Responsibilities
Maintaining patient confidentiality of patient/resident information and record security at all times
Completion of monthly/yearly statistics for home site
Preparation and processing of medical audits as deemed necessary by the Medical Advisory Committee or sub-committee, physicians, or Health Information Management Staff
Scan, index, and quality-check paper health records into the Electronic Medical Record (EMR), ensuring documents are accurately associated with the correct patient, encounter, document type, and date.
Conduct audits of scanned and indexed documents to ensure completeness, accuracy, legibility, appropriate document classification, and compliance with departmental standards. Identify and correct errors as required.
Completing all Release of Information request types (medical, patient, legal, insurance, etc)
Revision of procedures to update various technical duties (update the departmental as well as assist in updating organizational policies & procedures mainly related to privacy, confidentiality and release of information areas. With every change, there will be new responsibilities/duties added to each staff member with regards to monitoring processes) when necessary
Participate in quality assurance program in order to help achieve maximum and accurate functioning in all areas of the Health Information Management Department
Continuously involved in customer support especially in the area of confidential release of health information
Assist with any new HIM related projects
Evaluate requests for information in keeping with hospital policies and other legal requirements
Ensure that all charts of discharged patients/residents are brought to the Health Information Management Department within 24-48 hours of discharge for scanning
Adhere to occupational Health and Safety Legislation and Unity Health’s Health and Safety Policies
Adhere to Unity Health’s Policies and Procedures including Incident Reporting Policy
Actively demonstrates the Mission and Values set forth by Unity Health Toronto
Complete other duties as assigned
Qualifications
Two (2)
year Diploma in Health Information Management obtained from a currently recognized academic College by Canadian Health Information Management Association
Certified as a Health Information Management Professional (CHIMP)
Active CHIMA member in good standing and maintained on an annual basis
3-4 years of experience as a Health Information Management Professional handling the work area listed above in a similar facility
Proficiency with EPIC EMR system
Excellent working knowledge of Microsoft office package- Word, Excel, Access, and Meditech
