This position is dedicated to supporting older adults and individuals with disabilities by assessing their needs and implementing effective service plans. Your engagement will ensure clients can remain in their preferred settings while accessing vital community services across Clark, Cowlitz, Wahkiakum, Klickitat, and Skamania Counties.

Key Responsibilities:
• Perform client assessments and implement service plans
• Authorize and coordinate in-home community services
• Implement evidence-based community programs




• Facilitate effective transitions between care environments
• Maintain accurate and confidential client records

Requirements:
• Master’s or Bachelor’s degree in behavioral or health sciences with relevant experience
• Valid driver’s license and own transportation
• Robust understanding of local resource options
• Ability to work independently and under pressure
• Proficient documentation and reporting skills
