NLOT Is a Mobile Outreach Team Of Specialized Nurses That Provide Timely Care And Assessment Of LTC Residents To Minimize Preventable Hospitalizations. NLOT Nurses

Support the LTC home team to manage acute health conditions

Support Hospital to LTC home transfers

Collaborate with local services and other outreach teams

Enhance the skills and capabilities of LTC staff to detect and manage acute changes in older adults with complex health conditions

The NLOT program is considered a service of the Specialized Geriatric Services (SGS) and Regional Geriatric Program (RGP) of Ontario entities. The hours of work would be currently days, however, would include the potential for weekends, evenings and statutory holidays, based on program needs (subject to change in accordance with operational requirements).

This position will work in off‑site community settings providing clinical care to support residents living in Long‑Term Care (LTC) homes.

Role
The role of the Registered Nurse is to work in collaboration and consultation with the NLOT colleagues, physicians, LTCH staff and system partners in the following key areas:





Build staff confidence with complex clinical procedures

Provide educational and clinical support, guidance, and coaching to LTC staff to help increase capacity to manage complex resident health conditions

Facilitate hospital‑LTCH repatriation and support the LTCH in meeting the needs of the resident (clinical or educational support) to facilitate transition back to the home

Provide education to meet home‑specific needs, in groups and individual one‑on‑one coaching

Promote the use of best practices

Facilitate educational opportunities aimed at supporting LTCH staff, residents and families

Embrace and adapt to change as the NLOT programs development and direction is agile and iterative, reflective of the dynamic nature of resident care needs and evolving Ontario Health and Ministry of Long‑Term Care priorities.

Essential Qualifications

Valid registration as a Registered Nurse with the College of Nurses of Ontario

Valid CPR certificate

A police record check completed within the last six (6) months from a Canadian Police Information Centre (CPIC), inclusive of vulnerable sector screening, will be required of the successful candidate

Previous relevant inpatient or community‑based experience: medical/surgical, , emergency department

Required travel within the catchment area

Advanced clinical skill set: IV insertion, nephrostomy tubes, drains, catheter insertions,



knowledge caring for g‑tubes, central lines, physical assessment skills, wound care, and palliative care.

Strong problem‑solving and critical thinking abilities, with talent for prioritizing tasks in complex situations

Ability to function as an independent practitioner and a highly effective team member

Ability to synthesis clinical information succinctly and accurately both written and verbally

Exceptional verbal and written communication skills with a variety of community and organizational partners

Demonstrated application of case management skills in complex clinical situations

Superior interpersonal and relationship building skills

Proficient computer skills (Word, Powerpoint, Excel)

Demonstrated ability to use best practice and research

Solid ability to communicate/transfer clinical summaries successfully

Proven ability to create and deliver education materials effectively

Preferred Qualifications

BScN

Experience in geriatric care

Project Management and Quality Improvement skills and experience

Experience with Cerner, Clinical Connect and Point Click Care

Immunization Requirements
