In this position, you will provide essential support and navigate clients through mental health and addiction services. Reporting to the Manager of Clinical Services, your role includes conducting assessments, creating care plans, and attending collaborative care meetings. Your expertise will help vulnerable individuals access the necessary resources within the community, promoting a holistic approach to mental wellness.
Key Responsibilities:
• Screen clients for appropriate mental health services
• Facilitate supportive counseling during program transitions
• Develop psycho-educational sessions based on client needs
• Coordinate with community agencies for resource navigation
• Attend care planning meetings for cohesive treatment
Requirements:
• Relevant University degree in mental health fields
• Two years recent mental health clinical experience
• Comprehensive knowledge of addictions and treatments
• Access to a reliable vehicle with a valid driver's license
• Strong teamwork and communication skills
