In this key role, you will work within the Behavioural Support Services Team to enhance care for clients experiencing dementia, addictions, and other challenges. You will facilitate collaboration among clients, families, and healthcare providers to implement effective Behavioural Support Plans and interventions.

Key Responsibilities:
• Create and implement individualized support plans
• Assess client behaviours and document findings
• Act as a liaison with clients, families, and care teams
• Offer training and support to family members and staff




• Perform crisis intervention and develop management strategies

Requirements:
• Post-secondary education in relevant fields
• 3+ years experience with mental health or similar populations
• Familiarity with psychosocial rehabilitation and recovery models
• Strong communication and interpersonal skills
• Access to a vehicle for travel within the Central LHIN
