As a full-time RPN, you will provide leadership in client-centered care while coordinating with Personal Support Workers. Your responsibilities include developing care plans, monitoring service delivery, and collaborating with interdisciplinary teams. Key Responsibilities:



• Conduct comprehensive assessments for effective care • Create and implement client-centered care plans • Mentor Personal Support Workers and Home Support Workers • Ensure quality assurance through regular service monitoring • Participate in quality improvement initiatives

Requirements: • Graduation from an accredited Licensed Practical Nursing program • Active registration with the College of Nurses of Ontario • At least 12 months’ experience in community health roles • Proven ability to handle HR responsibilities • Evidence of current CPR certification
