Stonehenge Therapeutic Community is seeking a compassionate Registered Practical Nurse to support addiction and mental health clients in the Guelph-Wellington area. Embrace a role that integrates healthcare and community support.
The position involves direct client care within a supportive setting focused on housing stability and enhanced quality of life. You will work alongside a dedicated multi-disciplinary team, conducting health assessments, crisis interventions, and providing patient education. This role also requires regular travel and participation in extended hours rotations.
Key Responsibilities:
• Evaluate and document clients' mental and physical health needs
• Implement nursing interventions and crisis responses




• Facilitate interdisciplinary care plan discussions
• Assist in managing medication and client education
• Participate in program development and quality improvement
Requirements:
• Registered Practical Nurse in good standing with CNO
• Minimum two years' experience in addictions or mental health
• Experience in crisis intervention and withdrawal management
• Reliable vehicle for client transportation
• Completion of vulnerable sector screening
