Enhance palliative care access in underserved communities as a Clinical Coach with the Central West Ontario Health Team. This role emphasizes education and implementation across diverse healthcare settings.
In this capacity, you will work closely with community organizations to implement the Ontario Palliative Care Competency Framework. Your focus will be on educating and mentoring front-line providers on the principles of palliative care, ensuring they are equipped to meet the needs of patients. Collaboration with the CW OHT leadership will be essential to drive practice change effectively.
Key Responsibilities:
• Deliver clinical education and training for providers
• Conduct mentorship of healthcare staff in various settings
• Engage in data collection for program evaluation
• Participate in quality improvement initiatives
• Collaborate with Indigenous communities as requested
Requirements:
• Relevant undergraduate degree in health discipline
• Regulated health professional registration in valuable standing
• Five years' experience in palliative care
• Demonstrated coaching and consultation skills
• Proficiency in Microsoft Office and educational technology
