Key Responsibilities:
• Offer nursing care aligned with the ACT fidelity framework
• Support clients in navigating health and housing systems




• Perform health assessments and provide education
• Participate in administrative tasks and record-keeping
• Facilitate patient connections to community resources

Requirements:
• Registered Nurse with CRNA certification
• Two years of experience in a nursing role
• Strong communication and advocacy skills
• Ability to engage with individuals in community settings
• Valid driver's license and insured vehicle for work
