Spectrum Health Care is seeking an Occupational Therapist to deliver community and virtual therapy services. Engage in assessments and create personalized intervention plans for clients.

In this role, you will provide ministry-funded therapy within clients' homes and virtually. You'll assess mobility, cognition, and safety, and develop evidence-based plans in collaboration with clients. Supervision of therapy assistants and effective communication with caregivers are key responsibilities in this community-driven position.

Key Responsibilities:
• Perform assessments on home modifications and mobility
• Develop customized fall prevention plans
• Collaborate on stroke rehabilitation and chronic condition management
• Supervise occupational therapy assistants effectively
• Connect clients with relevant community resources

Requirements:
• COTO registration in valuable standing
• Proficient documentation and communication skills
• Prior community practice experience preferred
• Valid driver’s license for community travel
• Familiarity with MS Office and clinical software
