Covenant House is seeking a dedicated Shelter Case Manager to lead comprehensive case management for at-risk youth. In this permanent position, you'll offer intensive support and develop individualized plans addressing housing, healthcare, and resilience needs. Your leadership within the Case Management Team will ensure high-quality service delivery while navigating youth through complex challenges.

Key Responsibilities:
• Provide intensive case management using a client-centred approach
• Develop therapeutic relationships to foster trust and engagement
• Assess youth needs and collaboratively create individualized case plans




• Facilitate access to community resources and supportive services
• Maintain accurate documentation and case notes for effective tracking

Requirements:
• BSW or MSW degree with 5 years relevant experience
• Registration with the College of Social Workers and Social Service Workers
• Expertise in crisis intervention and case management principles
• Strong commitment to trauma-informed, harm reduction practices
• Experience working with street-involved youth and complex challenges
