Role: Medical Social Worker for dialysis patients
Responsibilities
Assess psychosocial needs of patients and families
Provide counseling and emotional support
Coordinate community resources to assist with chronic kidney disease management
Collaborate with nephrologists, nurses, and dietitians to support patient care
Support treatment adherence and care transitions
Advocate for patient well-being
Qualifications & Skills
Strong interpersonal and communication skills
Ability to assess psychosocial needs and provide counseling
Team-oriented mindset with cooperative skills
Knowledge of chronic kidney disease and dialysis care preferred
Key Duties: Engage in patient advocacy, facilitate care coordination, and support emotional and social needs within a compassionate, team-based environment that emphasizes ongoing training and professional development.
