Key Responsibilities:
• Provide acute care in patient's homes as part of a virtual ward
• Act as MRP, coordinating care for assigned patients
• Collaborate with nurses, pharmacists,
and allied health professionals
• Document and manage care using PowerChart EMR
• Participate in rotating 12-hour shifts for coverage
Requirements:
• Nurse Practitioner certification with relevant education
• Experience in acute and community healthcare settings
• Robust interpersonal and clinical decision-making skills
• Familiarity with digital EMR solutions
• Commitment to patient-centered care in a home setting
