In this General Internal Medicine position, you will work intimately with patients, families, and an interprofessional team to enhance patient well-being. Your responsibilities will include conducting patient assessments, facilitating meetings, managing care plans, and liaising with community resources. This role also allows you to contribute to educational initiatives, guiding students and colleagues alike.
Key Responsibilities:
• Conduct assessments to identify service needs
• Implement intervention plans with patients and teams
• Facilitate discussions during family/team meetings
• Coordinate with community agencies for service access
• Provide clinical education to students and peers
Requirements:
• Master’s Degree in Social Work required
• Registration with the Ontario College of Social Workers
• Minimum three years hospital experience preferred
• Strong verbal and written communication skills
• Familiarity with ALC and community services
