Your responsibilities will include conducting assessments, managing medications, and educating families, while collaborating closely with healthcare professionals. Engage actively in resident care conferences to enhance service delivery.

Key Responsibilities:
• Implement person-centered care plans tailored to residents
• Maintain accurate assessments and documentation




• Assist with transitions for residents between care levels
• Educate families about health management
• Respond to significant health changes in residents

Requirements:
• Current registration as an LPN in your province
• Experience in senior care or long-term settings
• Excellent communication and coordination skills
• CPR and First Aid certification required
• Willingness to undergo a Vulnerable Sector Check
