Fraser Health is seeking a dedicated Relief Community Health Nurse at Langley Memorial Hospital. This part time role offers the opportunity to support frail clients in their community.
As a Community Health Nurse, you'll work to promote health and manage chronic conditions through direct care and education, addressing complex needs in a community setting. Collaboration with an interprofessional team is vital, ensuring clients receive optimal support and healthcare access.
Key Responsibilities:
• Build a rapport with clients for effective care
• Screen referrals and perform comprehensive assessments
• Develop and implement individualized care plans
• Coordinate client care transitions across services
• Maintain detailed clinical records and documentation
Requirements:
• Registration as a Registered Nurse with BCCNM
• At least one year of clinical experience with chronic conditions
• BC Driver's license and access to a vehicle
• Proven ability to communicate effectively
• Familiarity with community health resources and practices
