As an integral member of the Nephrology Program, you will utilize a chronic disease management approach and ensure patient readiness for transplant. Your role involves collaborating with transplant centers and managing referral processes to meet the unique needs of renal patients.

Key Responsibilities:
• Build relationships with renal patients and transplant centers
• Confirm readiness through required testing and follow-up
• Assist in ensuring social support for transplant candidates
• Monitor progress through the transplant work-up process
• Provide education and resources for patients and families

Requirements:
• RN registration with the College of Nurses of Ontario
• Two years of experience in nephrology
• Familiarity with renal replacement therapies
• Experience in patient education and assessment
• Robust interpersonal communication skills
