Program Overview The Eastern York Region North Durham (EYRND) Ontario Health Team (OHT) launched the Seniors Home Support (SHS) Program in 2021, an integrated primary care team serving frail, homebound seniors in the communities of Markham and Stouffville.
The program serves a roster of more than 300 patients who experience physical, cognitive, and social barriers to attending office-based care. Through its current model of care, patients receive longitudinal primary care, including routine and urgent home visits, virtual care, and palliative care. Improving access to in-home primary care is critical for this population: 50% live with significant cognitive impairment, 26% have a history of stroke, and 27% live with moderate to severe heart failure.
The interprofessional team includes family physicians, a nurse practitioner, a pharmacist, a chiropodist, registered nurses, community paramedics, and home care coordinators from partner organizations, including Markham Family Health Team (FHT), Seniors Home Support Physicians, Markham Stouffville Hospital (MSH), York Region Paramedic Services Community Paramedicine Program, and Ontario Health atHome. The SHS program is one of Ontario’s few home-based primary care programs that integrate community paramedics as part of the core care team.

Position:
We are presently seeking an enthusiastic individual to join our high functioning interprofessional team as a full-time Team Coordinator.
Reporting to the Executive Director, the incumbent will work within our Seniors Home Support Program serving the primary care needs of homebound seniors. The Team Coordinator provides administrative support for the SHS program and physicians.
Main Tasks, Duties and Responsibilities
Accounting & Data Support
· Track program expenses covered by the organization, reimbursement of fees and other expenses.
· Track program data using codes in the EMR and report results to the leadership team
Program Promotion
· Represent SHS and the team by educating patients and their families on the program and recognize at risk seniors.
· Liaise with community organizations including but not limited to: MSH, SEGOT, Ontario Health at Home (OHaH), York Region Paramedics.
· Manage and maintain patient and internal resources for SHS, which includes updating our resource documents.
Human Resources Support
· Coordinate SHS schedules including internal and external obligations via Accuro and Outlook calendars.
· Work with and manage EMR system for SHS staff- including; Creating, generating and maintaining scheduling templates for appointment booking consistency.
Organizational Support
· Receive referrals and connect with patients and families to introduce the program, provide information, and conduct an initial screening to determine program eligibility.
· Create new patient charts in the EMR and input patient demographics.
· Coordinate the patient enrollment process, providing status updates with the patient’s circle of care.
· Maintain patient demographic for effective team communication.
· Respond to program-related administration calls and inquires in a timely and professional manner.
· Coordinate appointment follow-ups and family meetings as indicated via tasks or as prompted by patient request.
· Liaise with community organizations, specialist offices, previous family MD offices, labs as required.
· File faxes within EMR
· Communicate patient updates to Primary Care Team, and vice versa.
· Plan and facilitate bi-weekly rounds and program steering committee meetings including sending virtual invitations, building and disseminating agendas, booking meeting rooms, facilitating rounds meetings, minute-taking and communicating action items.
· Organize supply orders as requested by clinical team.
· Coordinate seasonal vaccination campaign for enrolled patients
Tasks as indicated by SHS team including:
· Scanning documents
· Distribution of mail and other hard copy documents.
Qualifications and Education Requirements
· Minimum of Grade 12 education. Course in MOA is an asset.
· 1-2 years minimum in a health care environment.
· 1-3 years previous working experience. A background in medical administration is an asset.
· Computer competency: Windows, Microsoft Office suite, e-mail and internet research. Experience with Accuro EMR is an asset.
Required skills:
· High degree of accuracy and attention to detail (e.g. entering information into the EMR).
· Professional verbal and written communications.
· Problem solving and conflict resolution.
· Good judgment, willingness to take on new challenges.
· Teamwork and collaboration with interprofessional team.
· Proficiency in a second language is an asset.
Job Types: Full-time, Permanent
Pay: $45,000.00-$48,000.00 per year
